Anticorpii pot preveni sau limita infec\u021bia, elimin\u00e2nd agen\u0163ii patogeni prin efect de neutralizare. Cu toate acestea, nu to\u021bi pacien\u021bii dezvolt\u0103 o cantitate m\u0103surabil\u0103 de anticorpi dup\u0103 expunerea la virus, mai ales \u00een cazul infec\u021biilor asimptomatice sau cu manifest\u0103ri u\u015foare. Studiile \u0219tiin\u021bifice efectuate p\u00e2n\u0103 \u00een prezent, arat\u0103 c\u0103 \u00een unele cazuri, dup\u0103 infec\u021bie, anticorpii persist\u0103 doar o perioad\u0103 scurt\u0103; similar cu r\u0103spunsul imun din infec\u0163ia cu SARS-CoV-1 din 2003. R\u0103spunsul imun celular, pe de alt\u0103 parte, joac\u0103 un rol central \u00een ap\u0103rarea antiviral\u0103, limfocitele T cu memorie persist\u0103 c\u00e2\u021biva ani si confer\u0103 protec\u021bie \u00eempotriva reinfec\u021biilor severe1; 2; 3; 4.<\/p>\n<p>Detectarea limfocitelor T sensibilizate la virusul SARS-CoV2 reprezint\u0103 un alt element important \u00een <a href=\"https:\/\/www.synevo.ro\/oferte\/testul-real-time-pcr-pentru-diagnosticarea-covid-19\/\" target=\"_blank\" rel=\"noopener\" data-wpel-link=\"internal\">diagnosticul de laborator al COVID-19<\/a>. Celulele pacientului sunt stimulate cu peptide specifice, provenind din clivarea glicoproteinei de suprafa\u0163\u0103 a virusului (proteina S). Testul utilizeaz\u0103 un amestec de peptide din 2 zone: cap\u0103tul C-terminal (C-term) \u0219i cap\u0103tul N-terminal (N-term) (Fig. 1).<\/p>\n<p><img decoding=\"async\" class=\"alignnone size-medium wp-image-149567\" src=\"https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/gama-de-peptide-300x72.png\" sizes=\"(max-width: 300px) 100vw, 300px\" srcset=\"https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/gama-de-peptide-300x72.png 300w, https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/gama-de-peptide.png 500w\" alt=\"gama de peptide\" width=\"300\" height=\"72\" \/><\/p>\n<p>Figur\u0103 1. Stimularea se efectueaz\u0103 cu o \u201egama de peptide\u201d de 158 peptide suprapuse, de la cap\u0103tul N-terminal, respectiv 157 peptide de la cap\u0103tul C-terminal.<\/p>\n<p>Peptidele de la cap\u0103tul N-terminal au specificitate \u00eenalt\u0103 pentru SARS-CoV-2, \u00een timp ce cele de la cap\u0103tul C-terminal prezint\u0103 omologii mai mari cu alte beta coronavirusuri. O reac\u021bie izolat\u0103 fa\u0163\u0103 de peptidele de la cap\u0103tul C-terminal este asociat\u0103 cu reactivitatea \u00eencruci\u0219at\u0103 la alte beta coronavirusuri. Persoanele infectate cu SARS-CoV-2 (PCR-pozitiv) prezint\u0103 o reactivitate clar\u0103 at\u00e2t la peptidele de la cap\u0103tul N-terminal c\u00e2t \u0219i la cele de la cap\u0103tul C-terminal, chiar \u0219i la 5 luni dup\u0103 infec\u021bie.Figur\u0103 1. Stimularea se efectueaz\u0103 cu o \u201egama de peptide\u201d de 158 peptide suprapuse, de la cap\u0103tul N-terminal, respectiv 157 peptide de la cap\u0103tul C-terminal.<\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"alignnone size-medium wp-image-149566\" src=\"https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/limfocitele-T-catre-peptide-300x177.jpg\" sizes=\"(max-width: 300px) 100vw, 300px\" srcset=\"https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/limfocitele-T-catre-peptide-300x177.jpg 300w, https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/limfocitele-T-catre-peptide-600x354.jpg 600w, https:\/\/www.synevo.ro\/wp-content\/uploads\/2021\/04\/limfocitele-T-catre-peptide.jpg 672w\" alt=\"limfocitele T catre peptide\" width=\"300\" height=\"177\" \/><\/p>\n<p>Figur\u0103 2 Reprezentarea activ\u0103rii limfocitelor T de c\u0103tre peptide: Cel mai important semnal pentru activarea unei celule este recunoa\u0219terea specific\u0103 a unui peptid de c\u0103tre receptorul specific (TCR).<\/p>\n<h2>Recomand\u0103ri de testare<\/h2>\n<ul>\n<li>Suspiciune de infec\u021bie \u00een antecedente, f\u0103r\u0103 rezultat pozitiv la serologie;<\/li>\n<li>Pacien\u021bi cu PCR pozitiv, dar f\u0103r\u0103 anticorpi detectabili;<\/li>\n<li>Persoane vaccinate care nu au dezvoltat anticorpi.<\/li>\n<\/ul>\n<p><strong>Specimen recoltat<\/strong> \u2013 s\u00e2nge venos.<\/p>\n<p><strong>Recipient de recoltare<\/strong> \u2013 vacutainer cu heparinat de litiu; vacutainer f\u0103r\u0103 anticoagulant cu gel activator.<\/p>\n<p><strong>Volum proba<\/strong> \u2013 20 mL s\u00e2nge heparinat si 5 mL ser.<\/p>\n<p><strong>Stabilitate proba <\/strong>\u2013 s\u00e2ngele trebuie s\u0103 ajung\u0103 \u00een <strong>max. 24 ore <\/strong>la laboratorul la care se efectueaz\u0103 testul \u015fi \u00een aceasta perioad\u0103 se p\u0103streaz\u0103 la temperatura camerei. <u>Este contraindicat<\/u>\u0103<u> refrigerarea<\/u> <u>probei<\/u>.<\/p>\n<p><strong>Cauze de respingere a probei<\/strong>\u00a0\u2013 specimene care au dep\u0103\u015fit intervalul de stabilitate, probe refrigerate sau congelate.<\/p>\n<p><strong>Metoda<\/strong> \u2013 \u00eencorporarea unui nucleozid radioactiv (timidina) \u00een ADN-ul celular.<\/p>\n<h2>Interpretarea rezultatelor LTT \u2013 virusul SARS-COV-2<\/h2>\n<p>R\u0103spunsul imun celular mediat de limfocitele T confer\u0103 un grad de protec\u021bie fa\u0163\u0103 de virus. Aproximativ 10% dintre pacien\u021bii simptomatici \u015fi cca. 40% dintre cei asimptomatici prezint\u0103 niveluri sc\u0103zute de anticorpi dup\u0103 o perioada mai \u00eendelungat\u0103 de la infec\u0163ie. Demonstrarea reactivit\u0103\u021bii celulare fa\u0163\u0103 de virus r\u0103m\u00e2ne singura metoda de confirmare a infec\u021biei \u00een antecedente la ace\u0219ti pacien\u021bi. Persoanele infectate cu SARS-CoV-2 <a href=\"https:\/\/www.synevo.ro\/shop\/arn-viral-sars-cov-2\/\" target=\"_blank\" rel=\"noopener\" data-wpel-link=\"internal\">(PCR-pozitiv)<\/a> prezint\u0103 o reactivitate clar\u0103 la amestecul de peptide, rezultatele men\u0163in\u00e2ndu-se pozitive p\u00e2n\u0103 la 5 luni dup\u0103 infec\u021bie.<\/p>\n<p>Unele persoane neinfectate pot prezenta reactivitate doar fa\u0163\u0103 de peptidele de la cap\u0103tul C- terminal. Aceasta reactivitate arat\u0103 contactul anterior cu alte coronavirusuri endemice (229E, OC43, NL63 si HUK1). Unele studii, aflate \u00eenc\u0103 in derulare, par s\u0103 indice o imunitate par\u021bial\u0103 fa\u0163\u0103 de SARS-CoV-2 la ace\u0219ti pacien\u021bi, ceea ce se coreleaz\u0103 cu forme asimptomatice sau u\u0219oare la unii pacien\u021bi.
