La om mastocitele con\u0163in aproximativ 2-5 pg histamin\u0103\/celul\u0103, \u00een timp ce bazofilele prezint\u0103 o cantitate mai redus\u0103 (1 pg\/celul\u0103).<\/p>\n<p>Mediatorii mastocitari, inclusiv histamina, predomin\u0103 \u00een r\u0103spunsul alergic precoce; bazofilele reprezint\u0103 sursa major\u0103 de histamin\u0103\u00a0\u00een r\u0103spunsul alergic de faz\u0103 tardiv\u0103<sup>2<\/sup>.<\/p>\n<p>Histamina este sintetizat\u0103 \u00een aparatul Golgi prin decarboxilarea aminoacidului histidin\u0103 \u00een prezen\u0163a enzimei histidin-decarboxilaza. Histamina are o sarcin\u0103 electric\u0103 pozitiv\u0103 \u015fi se asociaz\u0103 cu\u00a0 grup\u0103rile carboxil ale proteoglicanilor din granulele mastocitelor. Dup\u0103 exocitoz\u0103, histamina disociaz\u0103 de proteoglicani la un pH neutru.<\/p>\n<p>O dat\u0103 eliberat\u0103 \u00een spa\u0163iul extracelular, histamina este metabolizat\u0103 rapid (timp de \u00eenjum\u0103t\u0103\u0163ire ~ 15-20 minute) prin metilare \u015fi oxidare. Metilarea, catalizat\u0103 de enzima histamin-metil-transferaz\u0103, constituie principala cale de metabolizare la nivelul c\u0103ilor respiratorii. Produsul ini\u0163ial, N-metilhistamina, poate fi excretat prin rinichi sau poate fi deaminat sub ac\u0163iunea monoaminoxidazei rezult\u00e2nd acidul 1,4 metil-imidazolacetic (MIIA). Histamina mai poate fi oxidat\u0103 si de c\u0103tre diamin-oxidaza (histaminaza) cu generarea acidului imidazolacetic (IAA).<\/p>\n<p>Datorit\u0103 metabolismului rapid este dificil s\u0103 se determine histamina \u00een plasm\u0103; \u00een schimb m\u0103surarea histaminei \u015fi a metaboli\u0163ilor s\u0103i \u00een urina din 24 ore constituie o alternativ\u0103 eficient\u0103 pentru testare ca urmare a faptului c\u0103 valorile acestora se men\u0163in mult mai mult timp crescute<sup>1<\/sup>.<\/p>\n<p>MIIA este principalul metabolit al histaminei (50% din histamin\u0103 este degradat\u0103 \u00een acest compus); din acest motiv reprezint\u0103 un marker cantitativ pentru eliberarea endogen\u0103 de histamin\u0103<sup>7<\/sup>. Astfel, MIIA poate fi util \u00een confirmarea diagnosticului de anafilaxie<sup>6<\/sup>. De asemenea s-a constatat c\u0103 nivelurile urinare de acid metil-imidazol acetic se coreleaz\u0103 str\u00e2ns cu severitatea mastocitozei sistemice \u015f pot constitui un intrument util at\u00e2t pentru confirmarea diagnosticului c\u00e2t \u015fi pentru aprecierea evolu\u0163iei \u015fi a prognosticului<sup>3<\/sup>.<\/p>\n<p>Unele forme rare de tumori carcinoide gastrice se manifest\u0103 clinic printr-un tablou atipic mediat prin histamina eliberat\u0103 de c\u0103tre celulele tumorale; la ace\u015fti pacien\u0163i\u00a0determinarea de acid metilimidazol acetic poate fi util\u0103, \u00eempreun\u0103 cu cea de gastrin\u0103 \u015fi acid 5-hidroxi indolacetic<sup>5<\/sup>.<\/p>\n<h2><strong>Recomand\u0103ri pentru determinarea <\/strong><strong>acidului 1,4 \u2013 metilimidazolacetic<\/strong><\/h2>\n<p>Evaluarea pacien\u0163ilor cu suspiciune de anafilaxie sau de mastocitoz\u0103 sistemic\u0103<sup>3;6<\/sup>.<\/p>\n<h2><strong>Preg\u0103tire pacient <\/strong><\/h2>\n<p>Se va evita, cu 2 zile \u00eenaintea recolt\u0103rii, consumul de alimente cu un con\u0163inut ridicat \u00een histamin\u0103<sup>1<\/sup>.<\/p>\n<p><strong>Recipient de recoltare <\/strong>\u2013 vas de 2-3 litri \u015fi pahar de plastic de unic\u0103 folosin\u0163\u0103 pentru urin\u0103, pe care se noteaz\u0103 cantitatea total\u0103 de urin\u0103 din 24 ore<sup>4<\/sup>.<\/p>\n<p><strong>Cantitate recoltat\u0103 <\/strong>\u2013 10 mL<sup>4<\/sup>.<\/p>\n<p><strong>Cauze de respingere a probei <\/strong>\u2013 specimen care nu a fost p\u0103strat la 2-8\u00b0C; utilizarea de substan\u0163e conservante (produc interferen\u0163e)<sup>4<\/sup>.<\/p>\n<p><strong>Prelucrare necesar\u0103 dup\u0103 recoltare <\/strong>\u2013 \u00een e\u015fantionul recoltat se vor ad\u0103uga c\u00e2teva pic\u0103turi de acid acetic glacial<sup>4<\/sup>.<\/p>\n<p><strong>Stabilitate prob\u0103 <\/strong>\u2013 1 lun\u0103 la -20\u00b0C<sup>4<\/sup>.<\/p>\n<p><strong>Metod\u0103<\/strong> \u2013 cromatografie de lichide cuplat\u0103 cu spectrometrie de mas\u0103 (LC\/MS)<sup>1<\/sup>.<\/p>\n<h2><strong>Valori de referin\u0163\u0103 Acid 1,4\u2013metil-imidazolacetic (metabolit histamina)<\/strong><\/h2>\n<p>&lt;3.8 mg\/24 h<sup>4<\/sup>.<\/p>\n<div><strong>Limite \u015fi interferen\u0163e<\/strong><\/div>\n<div>Ob\u0163inerea unei valori normale MIIA nu exclude un eveniment anafilactic<sup>6<\/sup>.<\/div>\n<p>&nbsp;<\/p>\n<h2><strong>Bibliografie<\/strong><\/h2>\n<div>1.
