{"id":217,"date":"2025-06-11T12:36:04","date_gmt":"2025-06-11T12:36:04","guid":{"rendered":"https:\/\/analizesanocare.ro\/?post_type=product&#038;p=217"},"modified":"2025-08-08T13:57:37","modified_gmt":"2025-08-08T13:57:37","slug":"adiponectina","status":"publish","type":"product","link":"https:\/\/analizesanocare.ro\/en\/analiz\u0103\/adiponectina\/","title":{"rendered":"Adiponectina"},"content":{"rendered":"<div class=\"info-content content-informatii-generale\">\n<h2><strong>Informa\u0163ii generale<\/strong> <strong>Adiponectina<\/strong><\/h2>\n<p>Adiponectina este un hormon polipeptidic cu greutate molecular\u0103 de 30kDa, secretat de adipocite, cu efect antiaterogenetic, antiinflamator, cardioprotectiv \u015fi, de asemenea, cu rol \u00een reglarea sensilibilit\u0103\u0163ii la insulin\u0103. Nivelele reduse ale adiponectinei au fost asociate, at\u00e2t \u00een modelele animale c\u00e2t \u015fi umane, cu diferi\u0163i factori de risc aterosclerotici: hipertensiune arterial\u0103, diabet zaharat de tip 2, rezisten\u0163a crescut\u0103 la insulin\u0103, nivel ridicat de trigliceride \u015fi sc\u0103zut de HDL, obezitate, dar \u015fi cu inflama\u0163ie.<\/p>\n<p>Spre deosebire de <a href=\"https:\/\/www.synevo.ro\/shop\/leptina\/\" target=\"_blank\" rel=\"noopener\" data-wpel-link=\"internal\">leptin\u0103<\/a>, adiponectina nu modific\u0103 aportul alimentar, ci determin\u0103 reducerea greut\u0103\u0163ii corporale prin stimularea consumului energetic \u015fi sc\u0103derea nivelului plasmatic al glucozei \u015fi lipidelor. Concentra\u0163ia seric\u0103 de adiponectin\u0103 este reglat\u0103 de acumularea de gr\u0103simi \u00een corp, \u00een special gr\u0103simea visceral\u0103, hormonul av\u00e2nd un nivel plasmatic mai sc\u0103zut la indivizii obezi.<\/p>\n<p>Pacien\u0163ii supraponderali cu rezisten\u0163\u0103 la insulin\u0103 nu beneficiaz\u0103 de medicamente care stimuleaz\u0103 secre\u0163ia de insulin\u0103 (insulinsecretagoge), ci de tratament cu sensibilizatori ai insulinei care duc la o cre\u015ftere m\u0103surabil\u0103 a secre\u0163iei de adiponectin\u0103. Astfel, adiponectina poate fi folosit\u0103 ca un parametru de control \u00een cazul terapiei cu sensibilizatori ai insulinei.<\/p>\n<p>Numeroase studii efectuate\u00a0\u00een ultimii ani, at\u00e2t pe culturi de celule, c\u00e2t \u015fi pe modele animale \u015fi umane au avut ca subiect implicarea adiponectinei \u00een procesele fiziologice \u015fi patologice. In urma acestor cercet\u0103ri s-a eviden\u0163iat faptul c\u0103, la nivel vascular, adiponectina are efect protector, prin diminuarea fenomenelor inflamatorii asociate cu ateroscleroza, prin reducerea expresiei moleculelor de adezivitate celular\u0103 (selectina, vascular adhesion molecule \u2013 VCAM1, intercellular adhesion molecule \u2013 ICAM1) \u015fi a citokinelor at\u00e2t la nivelul endoteliului, c\u00e2t \u015fi \u00een macrofage.<\/p>\n<p>\u00cen cursul procesului de producere a pl\u0103cii de aterom macrofagele elibereaz\u0103 diferite citokine, ce determin\u0103 proliferarea \u015fi migrarea patologic\u0103 a celulelor musculare netede vasculare, iar acest fenomen este inhibat de adiponectin\u0103. De asemenea, \u00een studiile efectuate pe culturi celulare s-a aratat c\u0103 hormonul produs de adipocite scade nivelul esterilor de colesterol ce se acumuleaz\u0103 la nivelul histiocitelor umane, fenomen care, in vivo, joac\u0103 un rol important \u00een ini\u0163ierea leziunilor aterosclerotice.<\/p>\n<p>La nivel vascular, adiponectina activeaz\u0103 nitric-oxid-sintaza, enzim\u0103 ce determin\u0103 sinteza monoxidului de azot (factorul de relaxare derivat din endoteliu \u2013 EDRF) responsabil de relaxarea mu\u015fchiului neted vascular \u015fi inhibarea agreg\u0103rii plachetare.<\/p>\n<p>Studii pe modele animale au pus \u00een eviden\u0163\u0103 implicarea adiponectinei \u00een diverse afec\u0163iuni hepatice. La animalele de laborator, c\u0103rora li s-au indus hepatopatii de origine alcoolic\u0103 sau non-alcoolic\u0103, s-a constatat c\u0103 nivelul plasmatic al adiponectinei scade semnificativ. Administrarea intravenoas\u0103 de adiponectin\u0103 la aceste animalele a dus la reducerea hepatomegaliei, a fenomenelor inflamatorii \u015fi a steatozei, acest efect fiind cauzat \u015fi de capacitatea hormonului de a suprima produc\u0163ia hepatic\u0103 de TNF\u03b1.<\/p>\n<p>Efectele favorabile ale adiponectinei asociate cu efectele antiinflamatoare, antiapoptotice, inhibarea fibrozei intersti\u0163iale fac din acest hormon o molecul\u0103 terapeutic\u0103 promi\u0163\u0103toare<sup>1;2;3;4<\/sup>.<\/p>\n<h2><strong>Recomand\u0103ri pentru determinarea adiponectinei<\/strong><\/h2>\n<ul>\n<li>\u00a0<a href=\"https:\/\/www.synevo.ro\/ateroscleroza-profil-lipidic\/\" data-wpel-link=\"internal\">ateroscleroza<\/a>;<\/li>\n<li>obezitate;<\/li>\n<li>boli cardiovasculare;<\/li>\n<li><a href=\"https:\/\/www.synevo.ro\/diabetul-zaharat-tip-2\/\" data-wpel-link=\"internal\">diabet zaharat tip II<\/a>;<\/li>\n<li>monitorizarea tratamentului cu sensibilizatori ai insulinei <sup>2<\/sup>.<\/li>\n<\/ul>\n<h2><strong>Preg\u0103tire pacient<\/strong><\/h2>\n<p>\u00e0 jeun (pe nem\u00e2ncate)<sup>1<\/sup>.<\/p>\n<p><strong>Specimen recoltat <\/strong>\u2013 s\u00e2nge venos<sup>1<\/sup>.<\/p>\n<p><strong>Recipient de recoltare <\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator<sup>1<\/sup>.<\/p>\n<p><strong>Prelucrare necesar\u0103 dup\u0103 recoltare <\/strong>\u2013 se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C sau la -20\u00b0C<sup>1<\/sup>.<\/p>\n<p><strong>Volum prob\u0103<\/strong> \u2013 minim 1 mL ser<sup>1<\/sup>.<\/p>\n<p><strong>Cauze de respingere<\/strong> <strong>a probei <\/strong>\u2013 specimen hemolizat, icteric sau lipemic<sup>1<\/sup>.<\/p>\n<p><strong>Stabilitate prob\u0103<\/strong> \u2013 5 zile la 2-8\u00b0C; 3 luni la -20\u00b0C; nu decongela\u0163i\/recongela\u0163i<sup>1<\/sup>.<\/p>\n<p><strong>Metoda <\/strong>\u2013 ELISA<sup>1<\/sup>.<\/p>\n<h2><strong>Valori de referin\u0163\u0103 Adiponectina<\/strong><\/h2>\n<p>Valori mediane adul\u0163i:<\/p>\n<ul>\n<li>b\u0103rba\u0163i: 7 mg\/L<\/li>\n<li>femei: 8.5 mg\/L<\/li>\n<\/ul>\n<p>\u00cen func\u0163ie de valorile ob\u0163inute se poate evalua riscul pentru rezisten\u0163a la insulin\u0103 \u015fi arterioscleroz\u0103:<\/p>\n<div>\u00a0&gt; 10mg\/L: risc sc\u0103zut<\/div>\n<div>7-10 mg\/L: normal<\/div>\n<div>\u00a04- 7 mg\/L: risc crescut<\/div>\n<div>\u00a0&lt; 4mg\/L: risc foarte mare<\/div>\n<div><\/div>\n<h2><strong>\u00a0<\/strong><strong>Interpretarea rezultatelor<\/strong><\/h2>\n<p>\u00cen general, nivelele sc\u0103zute de adiponectin\u0103 sunt asociate cu o prevalen\u0163\u0103 crescut\u0103 a bolilor cardiovasculare asociate cu obezitatea. Cu toate acestea, rolul adiponectinei ca factor predictiv \u00een tulbur\u0103rile cardiovasculare r\u0103m\u00e2ne controversat, fiind necesare studii ulterioare pentru a demonstra leg\u0103tura de cauzalitate dintre nivelul adiponectinei \u015fi dezvoltarea bolilor cardiovasculare.<\/p>\n<p>Studii experimentale arat\u0103 c\u0103 deficitul de adiponectin\u0103 contribuie la exacerbarea remodel\u0103rii cardiace \u015fi la stimularea proceselor inflamatorii \u00een condi\u0163ii patologice. Nivelul plasmatic de adiponectin\u0103 cre\u015fte ca r\u0103spuns la inflama\u0163ia sistemic\u0103 \u015fi determin\u0103 inhibarea proceselor inflamatorii prin modificarea aderen\u0163ei monocitelor \u015fi prolifer\u0103rii celulelor musculare netede. Hiperadiponectinemia se asociaz\u0103 cu boli inflamatorii cronice.<\/p>\n<p>Concentra\u0163iile plasmatice sc\u0103zute de adiponectin\u0103 reflect\u0103 un stadiu avansat de ateroscleroz\u0103. La pacien\u0163ii cu infarct miocardic acest nivel poate fi neutralizat de cre\u015fterea adiponectinei declan\u015fat\u0103 de inflama\u0163ia sistemic\u0103, care de multe ori se asociaz\u0103 cu infarctul miocardic.<\/p>\n<p>Valorile serice al adiponectinei sunt \u00een direct\u0103 leg\u0103tur\u0103 cu gradul de sensibilitate la insulin\u0103 \u015fi scad semnificativ la pacien\u0163ii obezi cu insulinorezisten\u0163\u0103, toleran\u0163\u0103 alterat\u0103 la glucoz\u0103 \u015fi afec\u0163iuni coronariene.<\/p>\n<p>Unele studii efectuate arat\u0103 c\u0103 adiponectina\u00a0variaz\u0103 invers propor\u0163ional cu glicemia, insulinemia, trigliceridemia \u015fi direct propor\u0163ional cu nivelul HDL-colesterolului \u015fi metabolizarea glucozei mediat\u0103 insulinic, valorile plasmatice sc\u0103zute ale adiponectinei reprezent\u00e2nd un poten\u0163ial marker al insulinorezisten\u0163ei<sup>2,4,5<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2><strong>Bibliografie<\/strong><\/h2>\n<div>1.
