{"id":207,"date":"2025-06-11T12:12:19","date_gmt":"2025-06-11T12:12:19","guid":{"rendered":"https:\/\/analizesanocare.ro\/?post_type=product&#038;p=207"},"modified":"2025-08-08T13:57:55","modified_gmt":"2025-08-08T13:57:55","slug":"acizi-biliari-in-materii-fecale","status":"publish","type":"product","link":"https:\/\/analizesanocare.ro\/en\/analiz\u0103\/acizi-biliari-in-materii-fecale\/","title":{"rendered":"Acizi biliari in materii fecale"},"content":{"rendered":"<h2><b>Informa\u021bii generale Acizi biliari in materii fecale<\/b><\/h2>\n<p>Determinarea de <strong>acizi biliari in materii fecale <\/strong>este recomandat\u0103 pentru diagnosticarea <a href=\"https:\/\/www.synevo.ro\/sindromul-de-colon-intestin-iritabil\/\" data-wpel-link=\"internal\">sindromului de colon iritabil<\/a>.<\/p>\n<p>Acizii biliari sunt produse naturale ale sintezei colesterolului care ajut\u0103 la emulsificarea \u0219i absorb\u021bia gr\u0103similor alimentare din intestinul sub\u021bire. Majoritatea acizilor biliari sunt reabsorbi\u021bi \u00een ileonul individului s\u0103n\u0103tos, iar doar 5% excreta\u021bi \u00een fecale. Acizii biliari primari \u2013<b>acidul colic<\/b> (CA) \u0219i <b>acidul chenodeoxicolic<\/b> (CDCA) -sunt deconjuga\u021bi \u0219i dehidroxila\u021bi prin bacterii intestinale \u00een acizi biliari secundari \u2013 <b>acid deoxicolic<\/b> (DCA) \u0219i respectiv <b>acid liticolic<\/b> (LCA).<\/p>\n<p>Suma CA, CDCA, DCA, LCA \u0219i acid ursodeoxicolic (UDCA) reprezint\u0103 majoritatea acizilor biliari din fecale. Absorb\u021bia deficitar\u0103 a acizilor biliari \u00een ileonul terminal duce la un exces de acizi biliari \u00een colon care pot provoca <a href=\"https:\/\/www.synevo.ro\/sindromul-diareic-forme-de-diaree\/\" data-wpel-link=\"internal\">diaree<\/a> din secre\u021bia de clorur\u0103 \u0219i ap\u0103, o afec\u021biune numit\u0103 malabsorb\u021bie a acizilor biliari (MAB)<sup>1,2<\/sup>.<\/p>\n<p><b>Sindromul intestinului iritabil (SII)<\/b> este o tulburare nespecific\u0103 multifactorial\u0103 care implic\u0103 intestinul gros. IBS se caracterizeaz\u0103 prin dureri abdominala recurente, legate de actul defecatiei, modificari ale frecventei si consistentei scaunelor, prezente de cel putin 6 luni, conform criteriilor ROME IV. S-a observat ca p\u00e2n\u0103 la 50% dintre pacien\u021bii cu IBS-D prezinta un tranzit accelerat al colonului<sup>2<\/sup>.<\/p>\n<p>Cuantificarea acizilor biliari fecali ajut\u0103 la depistarea IBS-D \u0219i identificarea pacien\u021bilor cu diaree cronic\u0103 care pot beneficia de terapia sechestrant\u0103 a acizilor biliari<sup>3<\/sup>.<\/p>\n<h2><b>Recomand\u0103ri pentru determinarea acizilor biliari din materiile fecale<\/b><\/h2>\n<p>Pentru evaluarea pacien\u021bilor suspecta\u021bi de simptome ale sindromului intestinului iritabil \u2013 diaree din cauza malabsorb\u021biei acizilor biliari<sup>2,3<\/sup>.<\/p>\n<h2><b>Preg\u0103tire pacient<\/b><\/h2>\n<p>Pentru 3 zile \u00eenainte \u0219i \u00een timpul perioadei de colectare (regim special):<\/p>\n<ul>\n<li>Pacientul trebuie s\u0103 urmeze o diet\u0103 controlat\u0103 cu gr\u0103simi (100-150 g gr\u0103simi pe zi)<\/li>\n<li>F\u0103r\u0103 laxative (\u00een special ulei mineral \u0219i ulei de ricin)<\/li>\n<li>F\u0103r\u0103 \u00eenlocuitori de gr\u0103simi sintetici sau suplimente nutritive care blocheaz\u0103 gr\u0103simile<\/li>\n<\/ul>\n<p><b>Specimen recoltat\u00a0\u2013 <\/b>materii fecale<sup>3<\/sup><\/p>\n<p><b>Recipient de recoltare\u00a0\u2013 <\/b>coprocultor fara mediu de transport<\/p>\n<p><b>Prelucrare necesar\u0103 dup\u0103 recoltare \u2013\u00a0<\/b>Pacientul poate p\u0103stra proba la temperatura frigiderului \u00een timpul perioadei de colectare. Bariul interfereaz\u0103 cu procedura de testare, prin urmare se recomand\u0103 o perioad\u0103 de a\u0219teptare de 48 de ore \u00eenainte de analiza probei<sup>4<\/sup>.<\/p>\n<p><b>Volum prob\u0103 \u2013<\/b>1g<\/p>\n<p><b>Stabilitate prob\u0103 \u2013<\/b> 2 zile la temperatura camerei; 7 zile la 2-8\u00b0C; 24 de luni la -20\u00b0C<\/p>\n<p><b>Metod\u0103 \u2013\u00a0<\/b>ELISA<\/p>\n<p>Acizii biliari fecali frac\u021biona\u021bi sunt cuantifica\u021bi \u00eentr-o colec\u021bie fecal\u0103 de 48 de ore \u00een timpul c\u0103reia a fost urmat\u0103 o diet\u0103 bogat\u0103 \u00een gr\u0103simi.
