{"id":200,"date":"2025-06-11T11:59:52","date_gmt":"2025-06-11T11:59:52","guid":{"rendered":"https:\/\/analizesanocare.ro\/?post_type=product&#038;p=200"},"modified":"2025-08-08T13:58:05","modified_gmt":"2025-08-08T13:58:05","slug":"citochimie-esteraze-nespecifice","status":"publish","type":"product","link":"https:\/\/analizesanocare.ro\/en\/analiz\u0103\/citochimie-esteraze-nespecifice\/","title":{"rendered":"Citochimie esteraze nespecifice"},"content":{"rendered":"<div class=\"info-content content-informatii-generale\">\n<h2><strong>Informa\u021bii generale Citochimie esteraze nespecifice<\/strong><\/h2>\n<p>Citochimia se bazeaz\u0103 pe studiul microscopic al componen\u0163ilor chimici celulari: nucleoproteine, proteine, hidra\u0163i de carbon, lipide \u015fi enzime.<\/p>\n<p>Colora\u0163iile citochimice ofer\u0103 informa\u0163ii \u00een leg\u0103tur\u0103 cu linia celular\u0103 \u015fi diferen\u0163ierea celulelor hematopoietice \u015fi joac\u0103 un rol important \u00een evaluarea anomaliilor hematologice, \u00een special \u00een diagnosticul diferen\u0163ial \u015fi clasificarea bolilor maligne ale s\u00e2ngelui.<\/p>\n<p>Un principiu de care trebuie \u0163inut seama este faptul c\u0103, o reac\u0163ie pozitiv\u0103 este \u00eentotdeauna valoroas\u0103,\u00a0\u00een timp ce reac\u0163ia negativ\u0103 este neconcludent\u0103. Poate utiliza ca substraturi: alfa-naftil acetat, alfa-naftil butirat, naftol AS- D acetat<sup>4<\/sup>.<\/p>\n<p><strong>Principiul metodei<\/strong><\/p>\n<p>Ne vom referi la reac\u0163ia alfa-naftil acetat esteraza (ANAE): alfa-naftil acetatul este hidrolizat de esteraze \u00een alfa-naftol, care\u00a0\u00een prezen\u0163a unei s\u0103ri de diazoniu formeaz\u0103 un compus de culoare ro\u015fu-brun, insolubil\u00a0\u00een ap\u0103<sup>1;3<\/sup>.<\/p>\n<h2><strong>Recomand\u0103ri pentru determinarea esterazelor nespecifice<\/strong><\/h2>\n<ul>\n<li>diagnosticul LA monocitare tipurile M4 si M5<sup>1<\/sup>.<\/li>\n<\/ul>\n<h2><strong>Preg\u0103tire pacient <\/strong><\/h2>\n<p>\u00e0 jeun (pe nem\u00e2ncate) sau postprandial (dup\u0103 mese)<sup>3<\/sup>.<\/p>\n<p><strong>Specimen recoltat<\/strong><\/p>\n<p>a) s\u00e2nge capilar \u015fi\/sau aspirat medular \u2013 frotiuri<\/p>\n<p>b) se recomand\u0103 recoltarea simultan\u0103 de s\u00e2nge venos pentru hemogram\u0103<sup>3<\/sup>.<\/p>\n<p><strong>Cauze de respingerea probei<\/strong><\/p>\n<ul>\n<li>frotiuri nefixate \u00een maxim 8 ore de la recoltare;<\/li>\n<li>frotiuri executate din s\u00e2nge recoltat pe EDTA<sup>3<\/sup>.<\/li>\n<\/ul>\n<p><strong>Recipient de recoltare<\/strong><\/p>\n<p>a) se execut\u0103 frotiuri de s\u00e2nge capilar direct pe lame de microscopie;<\/p>\n<p>b) vacutainer cu EDTA K3 (capac mov), pentru hemogram\u0103<sup>3<\/sup>.<\/p>\n<p><strong>Cantitate recoltat\u0103 <\/strong><\/p>\n<p>a) 3-5 frotiuri;<\/p>\n<p>b) 2 mL<sup>3<\/sup>.<\/p>\n<p><strong>Prelucrare necesar\u0103 dup\u0103 recoltare <\/strong><\/p>\n<p>Este preferabil ca fixarea frotiurilor s\u0103 se fac\u0103 \u00een primele 30 minute de la recoltare. Eventual, se poate face\u00a0\u00een primele 8 ore. Amestec fixator: alcool etilic 9 p\u0103r\u0163i + formol 40% 1 parte. Fixarea se cronometreaz\u0103 30 secunde, dup\u0103 care frotiurile se spal\u0103 cu jet de ap\u0103 de robinet. Frotiurile fixate se pot colora imediat sau se pot p\u0103stra c\u00e2teva zile la temperatura camerei, p\u00e2n\u0103 la colorare<sup>3<\/sup>.<\/p>\n<p><strong>Stabilitate prob\u0103<\/strong><\/p>\n<ul>\n<li>frotiurile nefixate sunt stabile maxim 8 ore la 18-30\u00baC;<\/li>\n<li>\u00a0frotiurile fixate sunt stabile 5 zile la temperatura camerei<sup>3<\/sup>.<\/li>\n<\/ul>\n<p><strong>Metoda <\/strong>\u2013 examinare microscopic\u0103<sup>3<\/sup>.<\/p>\n<h2><strong>Interpretarea rezultatelor<\/strong><\/h2>\n<div>In monocite, activitatea enzimatic\u0103 a ANAE se eviden\u0163iaz\u0103 prin apari\u0163ia unui compus ro\u015fu-brun, cu aspect difuz, moderat sau intens colorat (tip M \u2013 monocitar). In celulele T (inclusiv limfobla\u015ftii T) \u015fi megacariobla\u015fti, aspectul este mai mult focal, \u00een regiunea Golgi sau paranuclear. Aspectul color\u0103rii este util, \u00eentr-o anumit\u0103 m\u0103sur\u0103, \u00een diferen\u0163ierea tipurilor de celule.<\/div>\n<div><\/div>\n<div>Etapa de inhibare cu NaF a fost adaugat\u0103 pentru a face esteraza nespecific\u0103 \u201emai specific\u0103\u201d. Activitatea enzimei \u00een celulele liniei monocitare este inhibat\u0103 de NaF, \u00een timp ce \u00een restul celulelor activitatea persist\u0103.<\/div>\n<div>Alfa-naftil acetat esteraza este obi\u015fnuit negativ\u0103 \u00een celulele de serie granulocitar\u0103. Celulele blastice din LAM-M0, M1 \u015fi M2 sunt de obicei negative. In LAM\u2013M3 esteraza nespecific\u0103 este pozitiv\u0103 \u00een 20-30% din cazuri, fiind mai slab\u0103 ca intensitate decat \u00een monocite; reac\u0163ia este sensibil\u0103 la inhibare cu NaF \u00een aproximativ 1\/3-1\/2 din cazurile pozitive.<\/div>\n<div><\/div>\n<div>Celulele blastice monocitare din LAM-M4 au activitate ANAE intens pozitiv\u0103, difuz\u0103 (tip M) \u00een propor\u0163ie de peste 20%, iar \u00een cele din LAM-M5 \u00een propor\u0163ie de peste 80%, sensibil\u0103 la NaF.<\/div>\n<div>Megacariobla\u015ftii leucemici din LAM-M7 au pozitivitate focal\u0103 distinct\u0103 par\u0163ial inhibat\u0103 de NaF. Eritrobla\u015ftii din LAM-M6 au de obicei activitate ANAE pozitiv\u0103, neinhibat\u0103 de NaF.<\/div>\n<div>Celulele blastice din LAL sunt negative;\u00a0 foarte rar pot ap\u0103rea celule ANAE slab pozitive cu aspect granular, cu sensibilitate variabil\u0103 la NaF<sup>1;4<\/sup>.<\/div>\n<p>&nbsp;<\/p>\n<h2><strong>Bibliografie<\/strong><\/h2>\n<div>1.
